All cases involving complications, deaths, adverse or near-miss events must be logged each week in the M&M reporting system. Important near-miss events should also be logged. Cases for the prior Friday through Thursday evening must be logged by 12 p.m. on Friday each week. The individual involved in the case and/or the most senior resident on service is responsible for submitting these events. Residents should ensure the attending of record is aware of events. Events can also be reported by any team member.
